Surrey Young People’s Substance Misuse service (SYPSMS) is a county-wide specialist treatment service for young people aged up to 25. We offer free and confidential advice and support.

The screening tool and referral forms below will help you identify the most appropriate areas of concern with a young person and if substance misuse targeted or specialist support is indicated. Please complete all parts of the relevant form if making a referral.
